It seems to work for me both in the ada-mode from Emacs 21.4.1 and in
the current version. Maybe only some specific cases are broken?

By the way, the comment paragraph fill (M-q) does not work well for me,
(extra -- characters are left in the middle of comment lines) until I
change the custom value for ada-fill-comment-prefix:

everything seems to work fine. I have noticed that this default value
does work well when one uses always two spaces after the --. Is this
style prevalent so it justifies to have this value as default? Even in
such a case (this style is more frequent) it would be more safe to have
"-- " as default value because it will also work when having two spaces
in each comment line: the only problem will be that the resulting
comment paragraph will use only one space after each --.
